- SUMMARY - Engineered osteoblast cells were seeded on PEEK and CFRP vertebral cages. Cell adhesion and proliferation at 84 hours was five time higher on carbon composite than PEEK. The observation of the living cells directly on the cages showed a different distribution of the cells on the two different biomaterials. On the carbon composite cage the cells were homogeneously distributed as a continuous monolayer, while on the PEEK cage the cells were layered in a discontinuous mode, distributed in non-homogeneous ways, and associated in clusters.

- SUMMARY - Carbon fibers with a density close to bone for better stress transfer and electrical properties that enhance tissue formation potentially have multiple advantages when developing high-strength biomaterials. Results showed that for carbon-fiber-reinforced composite osseointegration in terms of percent bone area for distances from the implant surface of 0.1mm was four times higher compared to Titanium.

- SUMMARY - The ostaPek® cervical corpectomy cage constructs achieved primary stability of the anterior column, utilizing small fragments of bone, which reduces trauma at the graft harvest site. Bone bridging was observed throughout the implant in the standard x-ray. Initial fusion was achieved on all 50 patients. The implant seemed satisfactory for the following reasons: (1) The bio-material’s long carbon fibers are aligned in the sagittal plane, according to the normal trajectory of the dominant bone trabeculae, and the load that both a vertebral body and cage must bear. (2) This provides primary anterior column support. (3) Radiolucency with no artifacts for MRI and minimal artifacts for CT. (4) Ample space for morcelized autologous bone. A large segment of the iliac crest, with its known complications, is not required. (5) A convincing bone fusion through the cage can be observed, leaving a live bone bridge at the anterior column. (6) Long carbon fiber is flexible and should allow physiological load of the bone tissue over time.

- SUMMARY - In 73 of 75 cases, fusion was achieved. Only 2 cases could be discussed. No case of pseudoarthrosis has been observed. Fusion was assessed by a CT scan protocol at the 6th month with axial, coronal and oblique sagittal reconstruction slides at the same angle as the cages. The ELIF is a maximally effective and minimally invasive alternative to more traditional lumbar fusion operations achieving results which meet or exceed standard anterior and posterior fusions. The major assets are (1) respect of the anatomical structures, (2) no vascular or neural complications, (3) minimal blood loss (less than 50 cc in all patients), (4) shortened surgery time (mean time 105 min), (5) significant reduction of post-op pain and long term sequelae, (6) ability to utilize pedicle fixation through same incision, and (7) ability to perform procedure at L5-S1.

- SUMMARY - The ostaPek®ek PLIF cage and lumbar plating procedure was found to fuse spinal motion segments in a safe and reliable manner, remaining stable more than five years for a broad series of patients and pathologies. The ability to observe bone through or around the composite devices was an asset. Soft tissue could be observed adjacent to the implant with MRI. The long fiber composite’s varying strength, stiffness and anisotropic properties may be appropriate in more complex constructs of the spine.
